What new patients can expect on their first visit
On your first visit, you will need to fill out a form with symptoms and your basic health history. We advise all our patients to have the forms filled out prior to coming in. Please download and print the forms below for faster service.
At your appointment when we have all necessary information, you will have a consultation with the doctor to go over your concerns. Following that conversation, a specific exam and informational session may be advised depending on your symptoms and complaints.
